破折やカリエスがあまりにも大きく、歯質の保存が見込めない場合や歯周病に罹患した場合、「歯を抜かなくてはなりません。」という説明をする場合、

Doctor: “Unfortunately, we have to pull this tooth out.”

ドクター：「残念ながら、この歯は抜かなければなりません。」

と言います。別の表現方法として、

Doctor: “We have to remove this tooth.”

とか、

Doctor: “We have to extract this tooth.”

と、抜歯の専門用語である extract (名詞はextraction)という言葉も使います。

それでは何故、抜かなくてはならないかの説明ですが、

ドクター：「虫歯があまりにも大きく、クラウン（冠）をかぶせる歯の量が足りないし、歯の部分が丈夫ではありません。」と言う場合、

Doctor: “Your cavity is so big that there is not enough tooth structure to put a crown (cap) on.”

クラウンはcrownとか、一般用語として capと言ったりもします。

もしも歯が破折し、歯冠が崩壊していたり、歯冠がない場合、

Doctor: “Your tooth fractured at the neck, and there is nothing to support the crown.”

と言います。

もしも歯周病が原因で「歯を抜きます」と言いたい場合、

Doctor: “The jaw bone that supports the tooth (roots) got lost. There is not enough bone to hold the tooth.”

ドクター：「歯（根）をサポートするあごの骨がなくなり、歯が支えられません。」

また、このような表現でもいいと思います。

Doctor: “Your bone around the roots is gone so the tooth is very loose. Unfortunately, we need to remove this tooth.”

日本語では「骨が溶ける」という言い方をしますが、英語ではあまり「とける」(melt)という表現は使いません。もちろん、言っても意味は通じますが、あまりそういう表現はしません。
