カリエスには：

（１）脱灰

（２）エナメル質内のカリエス

（３）エナメルー象牙境を越えたカリエス

（４）歯髄が露出がしていて、歯髄炎にまで波及している場合の重度のカリエス

というように、進行度を患者さんに説明することは日常的に行います。それを英語でどのように表現すればいいのでしょうか。上記を４回のパートに分けて説明します。

（１）脱灰の場合

脱灰は英語で、”demineralization” と言います。

Doctor:  “You have a cavity, but fortunately, it’s at an early stage.”

ドクター：「虫歯がありますが、幸いに、まだ初期の段階です。」

Doctor:  “Do you see this white frosty area on your tooth surface?  This is called demineralization.  This happens  when the content of minerals in your teeth have decreased.”

ドクター：「歯の表面に真っ白になっているところがあるますね？　これは「脱灰」とというものです。歯のミネラルの量が少なくなっている証拠です。」

Patient:  “What does this do?  Will this color go back to its original tooth color?”

患者さん：「これはどうなるのですか。この色は元に戻りますか。」

Doctor:  “Once it’s frosty like this, it will stay like this or it could get worse.  If it gets worse, you could get a cavity, especially if nothing is done.  You may see a yellowish opening on your tooth surface which would need treatment.”

ドクター：「色が真っ白になった場合、このまま真っ白な状態になったままかひどくなって、虫歯になります。何もしなければ、黄色っぽい穴が歯の表面にできて、治療が必要です。」

Patient:  “Is there anything I can do?  What would you recommend?”

患者さん：「何かできることはありませんか。何をおすすめしますか。どうしたらいいですか。」

Doctor:  “When it’s still frosty, you can use toothpaste with fluoride so that the area mineralizes. Mineralization means that the fluoride goes on the tooth surface, creates a coating, and the teeth get stronger.  This is because the minerals from the fluoride protect the surface.  Also, cutting down on drinks with lots of sugar like soda and juice will help.”

ドクター：「歯の表面が真っ白の場合、フッ素入りの歯磨き粉の使用をおすすめします。そうすることによって、歯の表面がフッ素でコーティングされて、フッ素のミネラルによって歯が強くなります。あと、糖分の多い飲み物やジュースは控えた方がいいですね。」

Patient:  “Can I use regular toothpaste with fluoride?”

患者さん：「フッ素が入っている市販の歯磨き粉を使えばいいのですか。」

Doctor:  “I will prescribe you a toothpaste with higher concentration of fluoride today.  Please use that until your next check-up and clean-up appointment.  I will re-evaluate at your next check-up and clean-up appointment if you should continue using that or change to an over-the-counter toothpaste with fluoride.  We may even have to treat those areas, if it gets worse.”

ドクター：「フッ素がある程度高い濃度で入っている歯磨き粉の処方箋を今日、出します。次回の診査とメインテナンスのアポイントの時に、また再度チェックさせて下さい。その時の状態によって、処方箋レベルの歯磨き粉を使い続けて頂くか、市販のフッ素入り歯磨き粉に変更するか決めます。もしも虫歯になってしまったら、治療する必要があります。」

Patient:  “Okay.  I will try my best.”

患者さん：「わかりました。頑張ってみます。」
